Issue with photo – lever is installed incorrectly. The lever should be left of the door edge.
I would guess that the local fire inspector told them they needed a panic bar on the door and so they took one from another door without knowing how to rehand the trim. Looks like a hotel or apartment complex fitness room.
If i were going to guess, i would say the door originally came with the lower lock assembly to secure it. Since that space would be used by multiple clients, They needed to have something convenient for their clients to use. I am happy to see they at least used a Von Duprin exit device on it. I am not familiar enough with the outside trim to recognize it, assuming it is most likely a Schlage product. Would have been nice if they got the handing correct, RHR instead of LHR. I am guessing whoever installed it didn’t know the difference or how to change the handing. I think that if i had done that i would have corrected the handing and removed completely the Adams-Rite lock from the door and covered it. It would still pose a egress issue should someone lock it when the room was occupied. The exit device and corrected trim would have been more than adequate.
It’s all about the handing!

Lockset lever is oriented the wrong direction.
Well…the obvious answer is the lever being backward. Maybe a less obvious answer is there are only 2 hinges, which might result in the sagging that you see on the lockside gap.
Looks like a Hotel fitness center. Card access trim installed by mass installer that is not a true professional. Glass company installed door with deadbolt, no one thought to disable the deadbolt, possible life safety Issue, surprised that the inspector passed this installation

No Wall Stop on back wall for handle hitting the drywall (it’s not in the picture?)
Adam Rite narrow style latch, mounted on a wide style door. Which would be a problem with UL code for egress.
The obvious is, well, My rule #1 for hardware installs……..Read the instructions.
